By the end of this course, participants will be able to:

  • Apply advanced causal analysis techniques (root‑cause, systems-based) to complex incidents.
  • Use tools such as Fault Tree Analysis (FTA), AcciMap, and Safety Bow‑Tie to investigate failure mechanisms.
  • Incorporate human‑factors, organizational culture, and high‑reliability organization principles into investigations.
  • Evaluate the role of technology, including predictive analytics and AI in incident investigation.
  • Design and implement a safety investigation system that links to your Safety Management System (SMS).
  • Communicate investigation findings effectively, supporting corrective actions and learning loops.

Course Outline

Module 1: Foundations of Modern Incident Investigation

  • Incident causation models: from Swiss‑Cheese to systems thinking
  • The role of investigation in a High‑Reliability Organization (HRO)
  • Human factors, culture, and “just‑culture” principles
  • Legal, organizational & ethical dimensions of safety investigation
  • Exercise: Reflect on past incidents in your organization. Map them to causation models and discuss where system-level weaknesses may lie.

Module 2: System-Based Causal Analysis Techniques

  • AcciMap methodology for multi-layered causal analysis
  • Fault Tree Analysis (FTA): building fault trees and interpreting results
  • Bow-Tie analysis for risk controls and barrier management
  • Cause-and-Effect (Ishikawa) diagrams for structured brainstorming
  • Case Study: Work in groups to construct an AcciMap and fault-tree for a historical or hypothetical incident, then identify systemic corrective actions.

Module 3: Root Cause Techniques & Human Factors

  • Root Cause Analysis (RCA): advanced techniques beyond “5 Whys”
  • Human reliability analysis and performance variability
  • Organizational behavior: latent conditions, cultural causes, and system neglect
  • Interview techniques, witness statements, and data gathering best practices
  • Exercise: Conduct a mock investigation interview, apply RCA techniques, and identify latent root causes.

Module 4: Emerging Tools & Predictive Analytics for Investigation

  • Role of AI and machine learning in analyzing incident narratives and trends
  • Data‑driven pattern recognition: trend analysis, predictive models
  • Using Natural Language Processing (NLP) to classify incident reports
  • Building early-warning systems and feedback loops for proactive safety
  • Group Project: Design a proof-of-concept investigation workflow that uses predictive analytics or AI to flag potential root-cause signals.

Module 5: Investigation Reporting, Learning & Integration

  • Structuring investigation reports: from findings to recommendations
  • Communicating lessons: to management, staff, and stakeholders
  • Closing the loop: integrating learning into SMS, training, and processes
  • Monitoring and evaluating the effect of implemented corrective actions
  • Capstone Project: Each participant develops an investigation report + action plan for a real or hypothetical incident. Present to the cohort and get peer/facilitator feedback.

Course Administration Details:

METHODOLOGY

The instructor-led trainings are delivered using a blended learning approach and comprise presentations, guided sessions of practical exercise, web-based tutorials, and group work. Our facilitators are seasoned industry experts with years of experience, working as professionals and trainers in these fields. All facilitation and course materials will be offered in English. The participants should be reasonably proficient in English.

ACCREDITATION

Upon successful completion of this training, participants will be issued a Skillstate Institute certificate.

TRAINING VENUE

The training will be held at a Skillstate Institute Training Centre. The course fee covers the course tuition, training materials, two break refreshments, and lunch. All participants will additionally cater to their travel expenses, visa application, insurance, and other personal expenses.
